PURPOSE The Lukose & Jacob Scholarship Fund was established in 2013. The mission of the scholarship is to provide financial assistance to individuals enrolled for undergraduate or graduate study that have taken BLS (Basic Life Support), CPR (Cardio Pulmonary Resuscitation), or ACLS (Advanced Cardiac Life Support) through GearUpWithACLS or Training for Life. The Scholarship principally targets students in two and four year academic programs; however, students in certificate/licensing programs including but are not limited to fields such as paralegal training, IT, ultrasound techs, medical records personnel and nursing will also be considered.

FINANCIAL ASSISTANCE is based on financial need, academic performance, leadership potential, and participation in GearUpWithACLS or Training for Life BLS, CPR, or ACLS certification. Scholarships are awarded provided funds are available. The scholarship is not renewable.

SCHOLARSHIP AWARDS
The Lukose & Jacob Scholarship Fund awards scholarships using a comprehensive committee review process. Areas reviewed by the committee include, but are not limited to the following: academic accomplishments, community service, references, personal essay and financial need. The scholarship pays directly to recipient or towards the institution at the discretion of the awarding committee. The Scholarship is awarded without regard to race, color, ethnicity, gender or sexual orientation. Scholarships awarded are based upon the availability of funds in amounts ranging from 500.00 to 1000.00 USD.

1. DEADLINE for scholarship applications is Sunday, June 1, 2014. (NO EXCEPTIONS)
2. Refer to application process below for a list of the supporting documents needed (i.e., reference forms, evidence of GPA, etc.) Incomplete applications will not be considered.
3. If any question does not apply to you in this application please put N/A in the space.
4. Type or print legibly. Illegible applications will be returned to you.
5. You will be notified by phone or mail in May regarding the status of your application.

CRITERIA
• Applicants must have taken BLS, or CPR, or ACLS certification via an approved instructor of GearUpWithACLS or Training for Life
• Applicants must have a minimum GPA of 2.5 on a 4.0 scale.
• Applicants must demonstrate a need for financial assistance. Applicants must be a citizen or legal permanent resident of the United States.
• Student must file a Federal Student Aid Application (FAFSA) and submit a copy of their 2012-2013 Student Aid Report (SAR) at the time of scholarship application.

Application Process

SCHOLARSHIP APPLICANTS MUST PROVIDE:
• Completed application form.
• Official transcript in a sealed envelope from the institution.
• One recommendation from a faculty member, mentor or teacher.
• Demonstrated financial need as evidenced by the required submission of FAFSA's Student Aid Report (SAR).

SCHOLARSHIP AWARDS
• Award notification will be given by mid-August 2014. Award amounts will be distributed after a student's final grades are received for the Spring or Summer session.

Deadline for the application is Sunday, June 1, 2014. Applications postmarked after this date will not be considered.
